📋 Forms and Client Intake
Build questionnaires, consent forms, and intake forms right inside Foodzilla, then send them to clients and collect the answers straight onto their record. A simple builder gives you text fields, choices, ratings, scales, and e-signatures, and questions can show or hide based on earlier answers so each client only sees what's relevant to them. You can auto-send a form whenever you add a new client, and there are 23 ready-made templates to start from, including New Client Intake, Consent, Weekly Check-in, and Health History. Available on the Pro plan and above.
✨ Smart Forms That Do the Work for You
Some forms act on the answers the moment a client submits them. A Food and Symptom Diary works out the nutrition of each meal for you and can push those meals into the client's diary. The Meal Plan Preferences form fills in a client's likes, dislikes, and restrictions so the plan auto-generator matches them right away. There are also smart forms that log body measurements to a client's history and set up their calorie and macro goals automatically.
🔗 Public Forms and Lead Capture
Publish any form to a public link and share it anywhere, with no login needed. New enquiries land in your Leads tab, ready to convert into clients in one click. Public forms open with a branded header showing your photo, role, tagline, qualifications, and links, which you set once in Form Branding.
🖊️ Structured, Sign and Lock Notes
Write session notes from professional templates like SOAP and ADIME, then sign and lock them for an audit-ready record that can't be edited. Keep notes private or share them with the client, and attach files to keep supporting material in one place. Need to add something later? Addendums let you append dated notes without ever changing the original.
👤 Forms and Notes in the Client Portal
In their client portal, clients can fill in the forms you send and read the notes you share. They can also write their own notes back to you and attach files to them, and everything saves straight to their record.
✉️ Customisable Client Invitation Email
Personalise the email new clients receive when you invite them. Set your own subject line and welcome message, or leave them blank to use the default text. Available on the Pro plan and above.
📰 More Magazine PDF Options
Magazine-style meal plans now give you more control over the look. Pick a page background colour from beige, white, grey, darker tones, and soft colours, and hide the nutrition summary or back cover pages whenever you don't need them.
