Does Foodzilla Have Client Questionnaires?
Yes. Foodzilla includes built in client forms and questionnaires that you can create, customise, and send directly to clients.
You can use questionnaires for client intake, consent, check ins, food and symptom diaries, dietary preferences, discovery forms, and more.
Ready Made Form Templates
Foodzilla includes a library of ready made templates to help you get started quickly, including:

You can preview any template before using it. When you choose Use Template, Foodzilla creates a copy that you can customise for your own practice.
Create Your Own Questionnaire
You can also create a form from scratch:
- Go to Forms & Bookings
- Open the Forms tab
- Click New Form
- Add your questions
- Customise the form as needed
- Save and send it to your clients
Your own forms appear under Your Forms, where you can see how many questions they contain, how many times they have been sent, and how many responses have been completed.
Add Questions to a Booking
You can also collect information as part of the booking process.
- Go to Forms & Bookings
- Open Bookings
- Select a Booking Type
- Go to Booking Questions
- Click Add Question to create individual questions, or Use a Template to reuse an existing form
Client Name and Email are collected automatically as required booking details. You can then add any additional questions you need before the appointment.
Preview Before Sending
Forms and templates can be previewed so you can see exactly what the client will see before sending them.
This makes Foodzilla useful for collecting client information before consultations, monitoring progress between sessions, and keeping questionnaires connected with the rest of your client management workflow.
