Foodzilla Discovery Forms are designed for collecting information from prospective clients through a public form link. Anyone with the link can complete the form without logging in.
You can also automatically add someone as an active client as soon as they submit the form, helping streamline lead capture and client onboarding.
What Is a Discovery Form?
A Discovery Form creates a public shareable link that anyone can open and complete without needing a Foodzilla account.

How to Create a Discovery Form
- Go to Forms & Bookings.
- Create a new form.
- Enter the form title and optional description.
- Select Discovery as the form type.
- Add the questions you want to ask.
- Configure any payment, resource sharing, or client activation options.
- Click Save Form.
Once saved, Foodzilla creates a public link that you can share with prospective clients.
Automatically Add Someone as an Active Client
Turn on Add as an active client on completion if you want anyone who completes the Discovery Form to become an active client immediately.
When enabled:
• The person becomes an active client as soon as they submit the form
• They can access your Client Portal and mobile app
• They receive an email with instructions to set up their account
This removes the need to manually convert each new lead after they complete the form.
If you would rather review each submission first, leave this option turned off.

When Should I Use Add as an Active Client on Completion?
This option is useful when completing the form means the person is already ready to begin working with you.
For example, you might enable it for:
• New client onboarding forms
• Paid intake forms
• Coaching program signups
• Services where payment confirms enrolment
• Forms sent after a client has already agreed to work with you
For general enquiries or qualification forms, you may prefer to leave it disabled so you can review the lead before making them an active client.
Require Payment Before Form Submission
Discovery Forms can also require payment before the person can submit their answers.
Turn on Require payment to submit, then set the price and currency.

This can be useful for paid assessments, consultations, onboarding forms, or other services where payment should be collected before the form is completed.
Send a Free Resource After Submission
You can also turn on Send a free resource when this form is completed.
This lets you automatically send a recipe collection, meal plan, guide, download, or other resource after the form is submitted.

If Add as an active client on completion is turned off, Discovery Form submissions can remain as leads for you to review.
